Registration, Licensing and Accreditation of Social Welfare and Development Agencies (SWDAs) and Service Providers (SPs)

How can I use the DSWD eServices for Registration, Licensing and Accreditation of Social Welfare and Development Agencies (SWDAs) and Service Providers (SPs)?   1. Public User to register (Magrehistro ang Public User o aplikante)
  2. Public User to login and go to Registration, Licensing and Accreditation Form (Mag-login ang Public User at tingnan ang form para sa Pagpaparehistro, Paglilisensya at Akreditasyon)
  3. RLA Applicant to review the documents required before proceeding (Suriin ng RLA Applicant ang mga kailangang dokumento bago magpatuloy sa kasunod na hakbang)
  4. Applicant to fill up all required fields (Sagutan ng aplikante ang mga blangkong nangangailangan ng tugon)
  5. Applicant to upload scanned copies of required documents (Mag-upload ang aplikante ng scanned copies ng mga kailangang dokumento)
  6. Applicant checks submitted RLA application (Suriin muli ang kawastuhan ng aplikasyon bago isumite ng aplikante)
  7. DSWD Users to assess the submitted online application (Siguruhin ng DSWD User kung tuluyang naisumite na ang online application)
  8. Applicant receives notification of successful RLA application through email (Sa pamamagitan ng email, makakatanggap ang aplikante ng abisong nagsasabi na matagumpay na naisumite ang RLA application)
  9. Applicant receives notification through email on the status of initial assessment of application either Approve/Reject or Incomplete (Sa pamamagitan ng email, aabisuhan ang aplikante tungkol sa katayuan ng initial assessment sa kanyang aplikasyon, kung ito ba ay Aprobado / Na-reject / Hindi kumpleto)
  10. Applicants receives notification through email on the details of payment and the submission of required and other supporting documents (Sa pamamagitan ng email, aabisuhan ang aplikante tungkol sa detalye ng pagbabayad, kabilang na ang ibang kailangang pansuportang dokumento)
  11. Applicant receives notification through email the status of application after payment of processing fees such as: (Sa pamamagitan ng email, aabisuhan ang aplikante tungkol sa katayuan ng kanyang aplikasyon pagkatapos maibigay ang mga kailangang bayarin sa pagpoproseso ng mga ss:)

What is the timeline in processing Registration, Licensing and Accreditation online application?
  • Applications received are expected to be assessed within 24 working hours (3 days)
  • Any applications received from Mondays through Thursdays at 4:00 in the afternoon shall be acted the following day, likewise, applications received on Fridays at 4:00 in the afternoon shall be processed the next working day;
  • An automated notification will be forwarded to the applicant as to the status of the application

 

How much is the processing fee for Registration, Licensing and Accreditation?

The DSWD shall collect a processing fee for Registration, Licensing or Accreditation amounting to One Thousand Pesos (1,000.00) valid for three (3) years.

 

What do I need before I apply for Registration of Auxiliary Social Welfare and Development Agencies (SWDA)? [Ano ang mga kinakailangan para mag-apply sa pagpaparehistro bilang Auxiliary Social Welfare and Development Agencies (SWDA)?]

I. New Application: (Bagong Application)

  1. Accomplished Application Form (Nasagutang Application Form)
  2. Certified True Copy of Certificate of Registration and Articles of Incorporation & By-Laws (Certified True Copy ng Certificate of Registration at Articles of Incorporation & By-Laws)
    • SEC – for a Non-Stock, Non-Profit or Non-Stock Profit-Oriented Entity (SEC – para sa Non-Stock, Non-Profit o Non-Stock Profit-Oriented Entity)
    • CDA – for a Cooperative (CDA – para sa Kooperatiba)
  3. Updated Certification on the SWDA’s Status of Operation (applicable only if the date of registration with the concerned regulatory government agency is more than three (3) years prior to application) [Updated Certification ng Istatus ng Operasyon ng SWDA (importante lamang kung higit sa tatlong (3) taon nang nakarehistro sa regulatory government agency sa petsa ng aplikasyon bilang SWDA]
    • SEC – for a Non-Stock, Non-Profit or Non-Stock Profit-Oriented Entity SEC – para sa Non-Stock, Non-Profit o Non-Stock Profit-Oriented Entity)

  4. Work and Financial Plan for Two (2) Succeeding Years CY/FY [Work and Financial Plan para sa Dalawang (2) Magkasunod na Calendar Year (CY) o Fiscal Year (FY)]
  5. Manual of Operations/Handbook (Manual of Operations/Handbook)
  6. For those already operating prior to the application: (Para sa mga nakapag-paoperate na bago ang application:)
    • Annual Accomplishment Report of the previous year CY/FY [Annual Accomplishment Report ng nakalipas na Calendar Year (CY) o Fiscal (FY)]
    • Audited Financial Report of the previous year CY/FY [Audited Financial Report ng nakalipas na Calendar Year (CY) o Fiscal (FY)]
  7. For those operating in more than one region: (Para sa mga nag-o-operate sa higit sa isang rehiyon:)
    • Validation Assessment Report from the concerned DSWD-FO/s attesting to the existence and status of operation of the SWDA in the area/s of jurisdiction (Validation Assessment Report mula sa concerned DSWD-FO/s na nagpapatunay sa pag-iral at katayuan ng operasyon ng SWDA sa inilistang lugar na nasasakupan.)
    • List of main and satellite/branch offices to include the contact person/s, address/es and contact number/s and programs and services to be or being implemented (Listahan ng mga pangunahing tanggapan at satellite offices kasama ang contact person/s, mga adres nila, contact number/s, at mga programa at serbisyong ipinapatupad at pinaplanong ipatupad)
  8. For those operating in a tie-up scheme: (Para sa mga nag-o-operate ng tie-up scheme)
    • Memorandum of Agreement (MOA) or Contract of Partnership, among others between the applying agency and its partner agencies (Memorandum of Agreement (MOA) o Kontrata ng Partnership, bukod sa ibang pertinenteng dokumento na nagpapatunay sa pakikipagtulungan ng aplikanteng ahensya at partner nito)
  9. For those SWDAs with foreign national board member/s and/or employee/s: (Para sa mga SWDA na may banyagang miyembro ng board at/o empleyado)
    • Missionary Visa for each volunteer (Missionary Visa para sa bawat boluntaryo)
    • Working Visa for each paid staff (Working Visa para sa bawat sumusweldong kawani)

II. For Renewal: (Para sa pag-renew:)

  1. Original Registration Certificate that was previously issued (Orihinal na Registration Certificate na inisyu nang nakaraan.)

Validation assessment report from the DSWD FO/s where the expanded area/s of operation is/are located attesting to the existence and status of operation of the SWDAs (Validation assessment report mula sa DSWD FO/s na siyang sumasakop sa dineklarang pinalawak na lugar ng operasyon ng SWDAs, na siyang nagpapatunay na may operasyon nga ito sa nasabing lugar)
  3. Original Certificate of Registration (Orihinal na Certificate of Registration)
    • Accomplishment report of the previous year CY [Accomplishment report sa nakalipas na Calendar Year (CY)]
    • Audited financial report for the previous year CY/FY [Audited financial report ng nakalipas na Calendar Year (CY) o Fiscal Year (FY)]
    • Work and Financial plan for the two succeeding years CY/FY [Work and Financial plan para sa dalawang kasunod na Calendar Year (CY) o Fiscal Year (FY)]

IV. Transfer of Area/s of Coverage: (Paglilipat ng Saklaw na Lugar)

  1. Request for amendment of registration certificate (to be filed at the concerned DSWD Office that issued the current certificate) [Request o hiling na maamyenda ang sertipiko ng pagpaparehistro (na ipa-file sa concerned DSWD Office na naggawad ng kasalukuyang sertipiko)]

  • For residential or center-based: Annual Building Inspection Certificate or Sanitary Permit, Valid Fire Safety Certificate and Sanitary Permit or Water Potability Certificate (Para sa pasilidad na residensyal o center-based: Taunang Building Inspection Certificate o Sanitary Permit, Valid na Fire Safety Certificate at Sanitary Permit o Water Potability Certificate)

  5. Work and financial plan for the succeeding two (2) years CY/FY [(Work at Financial Plan para sa dalawang magkasunod na Calendar Year (CY) o Fiscal Year (FY)]
  6. Audited Financial Statement for the previous two (2) consecutive years CY/FY [Audited Financial Report ng nakalipas na Calendar Year (CY) o Fiscal (FY)]
  7. Accomplishment reports for two (2) years prior to the assessment (Accomplishment reports para sa dalawang (2) kasunod na taon bago ang assessment)
  8. Profile of client served/caseload inventory for the preceding and current year (Profile ng pinagsisilbihang kliyente/caseload inventory para sa nakaraan at kasalukuyang taon.)
  9. For residential care facilities and community center based facilities (Para sa pasilidad na residensyal o center-based)
  • Valid building structural safety certificate
  • Valid fire safety certificate
  • Valid water sanitation permit or water potability certificate
